Administration of Medicine/Treatment Form


There are two situations when we can give medicines at school:

1.  In case of long term complaints, such as asthma.
2.  When children are recovering from an illness, but are well enough to return to school and have a course of medicine to complete.

Only medicine that needs to be taken four times a day, or at a specific time of day should be brought into school and given to the Admin Officer, in a named bottle or syringe containing one dose only.  You will be asked to complete a form, giving permission for the medicine to be administered.  A form is available to download below or you can collect a paper copy from the school office.

If your child has asthma, you be given a card to complete.  The card should then be returned to school.
